Texas Chapter Wins 2013 John Tooker Evergreen Award
The John Tooker Evergreen Awards Program provides recognition and visibility to chapters that have successfully implemented programs to increase membership, improve communication, increase member involvement, enhance diversity, foster careers in internal medicine and improve management of the chapter. The Texas Chapter received a 2013 John Tooker Evergreen Award in the Medical Students category for the following program:
General Internal Medicine Statewide Preceptorship Program–a FOUNDATION for the Future
Texas ACP has run the General Internal Medicine Statewide Preceptorship Program (GIMSPP) for almost two decades. After critical state funding for the program was cut, Texas ACP rapidly formed a 501c3 tax-exempt corporation to ensure a Foundation for the Future for GIMSPP. The chapter’s leadership has committed the necessary resources to ensure the continued success of this very important program.
Texas Chapter Recognized as an Evergreen-All Star
In 1993, the Evergreen Awards Program was started as a way to recognize outstanding chapter efforts and innovation. As a way to celebrate the 20th anniversary of the program, the Chapters Subcommittee and staff looked for a way to recognize chapters who have demonstrated a long-term engagement with the program. The criteria included that a chapter must have submitted at least 5 nominations, had at least 2 winners, and had a nominated program running for at least 3 years. The Texas Chapter has contributed to the growth of all chapters through their commitment to sharing successful best practices through the Evergreen program and has been recognized as an Evergreen All-Star for their achievements.

Chapter Excellence Award
TXACP wins a 2012 Chapter Excellence Award for Texas ACP Northern and Southern Regions! The award recognizes chapters which successfully meet the standards for managing a chapter. In order to achieve the Chapter Excellence Award, chapters must meet all basic criteria and seven optional criteria. Criteria include such activities as formulating an effective Governor’s Council and committees, communicating frequently with membership, providing educational opportunities, recruiting and advancing members and celebrating membership through local awards. I would like to extend a special thanks to those chapter members who assisted me in all of these endeavors! For their hard work and dedication, we received this award.

Texas Medical Home Initiative Wins a 2012 John Tooker Evergreen Award at IM2012
Texas, like most of the nation, is experiencing a rapidly increasing population, a severe shortage of primary care physicians and an alarming number of residents with chronic medical problems including diabetes and obesity. For these reasons the Texas Chapter of the ACP embarked on a re-design of the business and health care delivery model for state residents. The Texas Medical Home Initiative emphasizes four key components: expanding patient access and engagement, improving chronic condition population management through use of patient registries, providing team-based care that focuses on care transitions and coordination, and monitoring performance on key quality/utilization measures. To work towards achieving these components, the TMHI focuses on providing practice support including assisting the practices in their transformation to the PCMH model and achieving NCQA recognition through use of an intensive coaching model, using technology to support optimal patient care, performance measurement, patient education and enhanced communication and providing timely and actionable patient-level clinical data. By engaging all stakeholders, sharing data and best practices transparently with the goal of delivering high quality, coordinated care in the Patient Centered Medical Home model, the Texas Chapter is experiencing success with their new initiative and they encourage other chapters to implement a similar program.

ACP Chapter Award Nominations to be Announced on November 3, 2012 at the TXACP Annual Meeting
Laureate Award
The Laureate Award honors those Fellows and Masters of the College who have demonstrated by their example and conduct an abiding commitment to excellence in medical care, education, or research and in service to their community, their Chapter, and the American College of Physicians.
The recipient of this award shall bear the title Laureate of the Texas Chapter.
These awards shall be presented at the Annual Scientific Meeting. The awardees should accept the recognition in person, unless excused by the Governor. The awardees are to be nominated by a process that may originate from the Awards Committee or from any member of the Chapter. The nomination must document the attributes and accomplishments of the nominee.
The awardees shall be senior physicians and Fellows or Masters of long-standing, with acknowledged excellence and peer approval in the field of internal medicine. In addition, the awardees should have served their Chapter with distinction. Exceptional circumstances may modify these requirements.
Texas Chapter Volunteerism and Community Service Award
The Texas Chapter Volunteerism and Community Service Award was launched in May 2000 to honor members of the Texas Chapter who have distinguished themselves in voluntary service in the area of medicine. Examples of volunteerism and community service include, but are not limited to community education on health and medical issues, medically related charity work, and community-based medical education.
Awardees must be Masters, Fellows, Members, or Associates of the College. The service must have been performed on a voluntary basis and not required for the completion of teaching, training, or position requirements. Volunteer work must have been done as a physician and be medically related.
Award recipients who are members (Masters, Fellows, or Members) of the College will be eligible for nomination for the Oscar E. Edwards Memorial Award for Volunteerism and Community Service presented by the national ACP each year at the College's Annual Session. Recipients of this award have distinguished themselves as true humanitarians to be honored for their voluntary contributions in medicine.
GIMSPP Decade of Service
The General Internal Medicine Statewide Preceptorship Program Decade of Service Award honors highly active and dedicated preceptors who have taught medical students in their practices for the past ten years. The award is presented to preceptors who have served as mentors to students for at least seven of those years. We thank these devoted preceptors for the time, knowledge, and energy they have unselfishly given to the future physicians of Texas.
TXACP Services Advocate of the Year Award
TXACP Services, the advocacy arm of the Texas Chapter of the ACP, is pleased to announce the creation of the Advocate of the Year award to be presented to a registered Key Contact who has demonstrated outstanding leadership in the areas of legislative advocacy and grassroots activism. This person has consistently communicated concerns on behalf of TXACP Services to elected officials on issues and unfailingly responded to legislative alerts, as well as served as a community spokesperson on behalf of internal medicine and patients. In addition, this person has expanded the Chapter¹s advocacy program, recruited new Key Contacts, and actively participated in Leadership Day visits and/or other meetings with legislators and staff.
TXACP Services Legislative Champion
The TXACP Services Legislative Champion award is presented in recognition of outstanding legislative leadership, demonstrated support for medical education funding and improved access to quality health care, sponsorship of TXACP Services legislation, and commendable voting records on issues of importance to medicine and to patients.
Nominations for Texas Chapter Awards
Nominations for Chapter awards should include a detailed letter of recommendation and curriculum vitae. Nominations for the Volunteerism and Community Service Award should include a detailed description of the program or activity for which the person is being nominated, and a description of the impact the program or activity has had on the target group or general community.
Additional letters of support are welcome and may strengthen the nomination.
